CoCM Initial (99492)
$165 for 70+ min in first month. Collaborative Care Model with psychiatric consultant and care manager.
CoCM Subsequent (99493)
$129 for 60+ min in subsequent months. Ongoing collaborative care management.
CoCM Add-on (99494)
$68 per additional 30 min (max 2). Add-on to 99492 or 99493 for extended care management sessions.
